The Community Accessability - One Network Program is funded by Victorian Department of Transport under the Flexible Local Transport Solutions Program.
The Program compliments existing community transport services in your community by providing eligible clients with the opportunity to book their transport online.
Eligibility
Clients must have a My Aged Care Referral or be eligible under NDIS or HACC PYP
Lives within the transport service catchment area
Not have access to a vehicle or unable to drive at present; or
Not have family or friends that can assist with transport
For new clients
Complete a contact form and one of our staff will contact you for further information.
For current clients
Once you have completed and submitted your form, you will receive a confirmation email advising we have received your trip booking request.
A confirmation SMS will then be sent to you 1 day prior to the trip date.
